Sick notes

If you’ve been off sick for more than 7 days

To request a sick note:

  • If you require a sick note for the first time, you will need to request an appointment with a clinician to discuss your health issue before a sick note can be issued. 

        Please submit a new Request and select                                            I want help with a health problem

 

  • If you require a repeat/continuation sick note, you will need to request this via our online form and select that you have had one previously. 

         Please submit a new Request and select                                            I have an Administrative query 

    

 

If you’ve been off sick for 7 days or less

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.
