Register with the Practice?

The practice area for Manor House Surgery covers Glossop including Simmondley, Gamesley, Charlesworth, Chisworth, Hadfield and Padfield.

The practice area for Manor House Hadfield covers Hadfield, Padfield, Tintwistle, Broadbottom, Hollingworth and Mottram.

Should you wish to register with the practice please come to the surgery and fill in the necessary forms. Alternatively click here to download a pdf registration form.

When you bring your registration form to the surgery you will be asked to fill in a new patient form about past medical history and will be asked to arrange an appointment with one of our nurses.

You will also need to fill in an ethnicity form (click here), as we are required to monitor the ethnic background of all patients registering with the practice.
